Art Life Images Patron Cash Terms and Regulations
  1. The Patron Cash New Sign-Up Account email address you submit must match an active Art Life Images website account email address. You can always edit your Patron Cash account profile to update or to make a correction.

  2. All Art Life Media, Inc., parent of Art Life Images, employees and form 1099 contractors are exempt from participating in the Patron Cash program.

  3. Rewards cannot be transferred to another Patron Cash ID account.

  4. This program is for US residents only, verified by the mailing address zip code where your cash pay-outs will be sent.

  5. All rewards are issued in dollars and sent via check 4 dates per year.
      a. 2007 rewards checks will be mailed:
        i. April 2, 2007
        ii. July 2, 2007
        iii. October 1, 2007
        iv. January 3, 2008

  6. Rewards cash cannot be posted to your Patron ID account until:
      a. All sales monies have been received by Art Life Images for sales attributed to your Patron Cash ID account.
      b. All attributed sale dates extend beyond the Art Life Images returns and refunds policy period.

  7. All Patron Cash ID account adjustments by Art Life Images must be made prior to any cash pay-outs to the Patron. Once Art Life Images has dispersed reward cash to the Patron, then Art Life Images has no right to request a refund from the Patron. Should a discrepancy be found, Art Life Images has the right to charge or credit a Patron's future account postings.

  8. Art Life Images reserves the right to end or change the Patron Cash rewards program at any time.

  9. Patron Cash rewards are taxable personal income. In order for a Patron Cash ID account holder to receive over $599.00 in cash rewards, the Patron must completely fill out, sign and fax a US W-9 form to Art life Images at 904-212-0821. The account for the current calendar year will be suspended until Art Life Images receives this completed and signed form.
      i. Click here to download a printable W-9 Request for Taxpayer ID Number and Certification form. You can fax it to us at 904-212-0821 at any time.
      ii. During a calendar year period, if you receive reward cash of over $599.00, Art Life Images will issue you the required 1099-MISC income statement for your IRS tax reporting needs.
      iii. If you do not receive cash rewards of over $599.00 then you will not receive a 1099-MISC income statement from Art Life Images.

  10. Art Life Images shall be the sole determiner in cases of suspected abuse, fraud or violation of its rules and any decision it makes relating to termination of Patron Cash ID accountholder (including forfeiture of pending rewards) shall be final and binding.

  11. Art Life Images reserves the right to close any Patron Cash ID account that has not been posted a reward in a contiguous 12 month period.
